City of Chicago Vaccination Mandate
Proof Of Vaccination Required For Indoor Dining
Per The City Of Chicago, effective 1/3/22, Gene & Georgetti will require every individual wishing to dine inside our building to show proof of vaccination against COVID-19.
-Individuals 5 years of age or older will be required to show proof that they are fully vaccinated. Valid forms of verification include: vaccination card, photocopy of vaccination card, digital record or app, or printed record from vaccine provider. Individuals ages 16+ must also present a valid photo ID.
-Test results will not be accepted in lieu of a valid vaccination card (with exception of medical or religious exemption outlined below).
-Individuals who will be in the building for less than 10 minutes for the purpose of ordering and carrying out food, making a delivery, or using the bathroom are exempt from this order.
-Individuals who have previously received a medical or religious exemption and hold proof of the medical or religious exemption as well as a COVID-19 test administered by a medical professional within the last 72 hours prior will be permitted to dine inside.
